Chapter Activities Award
(Covering June 1 through May 31)
Please submit reports to the National Block and Bridle Vice President and to the Block and Bridle Awards email.
This award is determined from the report submitted from the chapter. The report must be emailed by June 15 to be judged. There is no "Official Chapter Activities Reporting Form;" each school creates their own according to the guidelines listed below.
Submit your report with one activity per page under the categories listed below. If you are reporting more than one activity under a given category, simply duplicate the heading information and update the activity number (for example: "Education/Leadership Activities, Activity #3" for the third activity listed under this category).
Awards will be presented at the next National Block and Bridle convention. First through fifth place will be recognized with a plaque.

A. Cover page: Include the name of your school, "Block & Bridle Chapter Activities," and the year.

1. Chapter Name
2. College/University
3. Address

6. Number of initiates reported to the National Secretary during the school year

Following this should be places for the Club Advisor, Department Head, and Academic Dean to sign; include their names (typed) and telephone numbers.)
B. Table of Contents: Under each of the category headings (see below), list each activity and the page number it is on.
C. Activity Sheets: Each activity sheet should have the following:

All instructions must be followed for a chapter to be considered for the award.

- No less than a 12-point font may be used, all activity pages must be double-spaced, margins must be 1", and each activity is limited to a maximum of one page.
- Prioritize the listing within each category from most important to least important.
- All signatures (Chapter Advisor, Department Chair, and Academic Dean) MUST be obtained.
Date Due
Three copies should be submitted via email by June 15 to the National Vice President and the Block and Bridle awards email. For late entries, 3 points will be deducted for each day beyond the deadline (100-point scale), and entries will not be considered if they are two or more weeks late. The judging will be done during the summer and winners will be recognized at the next national Block & Bridle Convention.
